(f) To authorize the extension of a contract with Kemira
Water Solutions of Lawrence, KS, for the purchase of Ferric Sulfate for use at
the Wastewater Treatment Plant in the amount of $1.32/gal. for the estimated
usage of 53,000 gallons, approximately $70,000.
(g) To authorize the extension of a contract with Bonded
Chemicals, Inc., of Columbus, Ohio, for the purchase of a liquid phosphate
(Sodium Hexametaphosphate) for use at the Water Treatment Plant in the amount
of $5.52/gallon for the estimated usage of 7,400 gallons, approximately
$41,000.

LEGISLATION
1. Ordinance No. O2016-39, an ordinance changing the zoning
classification for approximately 14.5064 acres of land located at 2000 Germantown Road
and approximately 1.9169 acres of land located at 2400 Carmody Boulevard to a C-2 (Roadside
Commercial District). (2nd Reading)
2. Ordinance No. O2016-40, an ordinance establishing pay
ranges, pay policy and benefits for various salaried and hourly classifications
within the service of the City. (2nd Reading)
3. Ordinance No. O2016-41, an ordinance establishing pay
ranges and certain benefits for employee classifications in the Middletown
Department of Health and Environment. (2nd Reading)
4. Resolution No. R2016-39, a resolution authorizing the
filing of applications for CY 2017 with the Ohio Department of Transportation
for grants under the Ohio Public Transportation Grant Program and declaring an
emergency.
5. Resolution No. R2016-40, a resolution authorizing the
filing of an application with the Department of Transportation, United States
of America, for a grant under the Federal Transit Administration Act of 1991,
as amended and declaring an emergency.
6. Resolution No. R2016-41, a resolution to make adjustments
to appropriations for current expenses and other expenditures of the City of
Middletown, Counties of Butler and Warren, State of Ohio, for the period ending
December 31, 2016 and declaring an emergency. (General Fund)
7. Ordinance No. O2016-42, an ordinance establishing a
procedure for and authorizing a contract with Andritz Separation for the repair
of the cyclo gear of the centrifuge and declaring an emergency.
8. Ordinance No. O2016-43, an ordinance appropriating three
temporary easements, two perpetual easements to construct and maintain a sewer
and two fee simple interests in certain real property owned by AK Steel
Corporation and located at parcel numbers Q6542085000012, Q6542085000003 AND
Q6542085000014 (Project Parcels 78, 81 & 83), and declaring an emergency.
9. Ordinance No. O2016-44, an ordinance appropriating a
perpetual easement to construct and maintain a sewer and a temporary easement
in certain real property owned by Big Valley Properties, LLC located at parcel
number Q6542115000026 (Project Parcel 62), and declaring an emergency.
10. Ordinance No. O2016-45, an ordinance appropriating a
temporary easement in certain real property owned by Carol Bryant located at
parcel number Q6542102000037 (Project Parcel 7), and declaring an emergency.
11. Ordinance No. O2016-46, an ordinance appropriating two
temporary easements in certain real property owned by Yankee Road First Church
of God located at parcel numbers Q6542105000017, Q6542105000019, Q6542105000020,

12. Ordinance No. O2016-47, an ordinance appropriating a

21. Resolution No. R2016-44, a resolution to make
adjustments to appropriations for current expenses and other expenditures of

procedure for and authorizing an extension of the contract with ABL Management,

City Manager to enter an extension of professional services contract with
Kaiser Medical Corporation for medical and nursing services for incarcerated

procedure for and authorizing an extension of the lease of certain real

1040.03 of the Codified Ordinances levying and assessing sewer rates. (1st Reading)
29. Ordinance No. O2016-60, an ordinance amending Sections
